Typhoon Sendong (international name Washi) unleashed flash floods and landslides
on Cagayan de Oro, Iligan, Bukidnon, Compostela Valley, Zamboanga del Norte, Lanao
del Norte, Surigao del Sur, and Dumaguete on the evening of 16 December 2011. Per
the National Disaster Risk Reduction and Management Council, the latest death toll
as of 6 a.m. of 20 December 2011 stands at 957, with 338,415 persons affected.
In the wake of destruction brought about by Typhoon Sendong, the National Disaster
Risk Reduction and Management Council (NDRRMC), Department of Social Welfare and
Development (DSWD), and the Philippine Red Cross are prepared to accept donations for
the relief and rehabilitation efforts of the communities in the affected areas.

The Philippine Bankers Club and its member banks Banco de Oro, Bank
of the Philippine Islands, Metrobank, Philippine National Bank, and
Rizal Commercial Banking Corporation have agreed to waive the fees
on remittance to the abovementioned accounts.
To ensure that the much-needed aid will reach the intended
recipients at the soonest possible time, the Philippine Consulate
General strongly encourages everyone to send cash instead of in-kind
contributions.
Donations in kind
Groups or individuals who wish to send donations in kind are
requested to refer to the Revised Omnibus Guidelines in the Management
and Processing of Donations of the DSWD at
http://www.dswd.gov.ph/phocadownload/ao/ao6series2010.pdf for more
information. Under the guidelines, donations of used clothing are
strictly prohibited to safeguard the health of the people.
To facilitate the issuance of certification for duty free entry of
foreign donations, the registered, licensed and/or accredited Social
Welfare and Development Agency applicant shall submit three (3) copies
of the following documents to the concerned DSWD Field Office for
assessment at least two (2) weeks prior to the scheduled arrival of
donations in the country (copies will serve as file of DSWD-Field
Office, Standards Bureau and receiving copy for DOF).

Authenticated
Deed of Donation from the Philippine Consular Office of the country
of origin, with the accompanying certificate as to fitness of food
and medicine for consumption and compliance with the country of
origin’s sanitary and phytosanitary standards.

A separate Notarized Deed of Acceptance.

Copy of valid DSWD Registration, License and/or Accreditation Certificate.

Shipping documents such as original and/or duly certified true copy
of Bill of Lading or Airway Bill and Packing List.

Notarized Plan of Distribution conformed by the Local Social Welfare and
Development Officer, certified and endorsed by the DSWD Field Office(s)
that has jurisdiction over the target area for distribution.

In case of medicines, a certification of the Department of Health (DOH)-Bureau
of Food and Drugs (BFAD) that the commodities are allowed for free distribution
and use by the intended beneficiaries without the need of a prior prescription
and that these are safe for human consumption.

Notarized distribution report on latest shipment certified correct by the
concerned DSWD Field Office with attached Relief Distribution Sheets and
photo documentation.

In case of relief items other than food and medicines, proof of prior
agreements or approved arrangements with appropriate government
agencies

Donations from Hong Kong
In an effort to provide the public with information on the relief efforts
that have been undertaken by the Filipino community in Hong Kong in response
to the massive floods brought by Typhoon Sendong (Washi) in Mindanao, the
Philippine Consulate General will set up a Web page to indicate the kind and
quantity of donations that have been directed to the Philippine Government
and various relief organizations in the Philippines.
The information to be contained in this advisory will be derived from reports
submitted by community members to the Philippine Consulate General. For any
