REQUIREMENTS FOR CONTRACT VERIFICATION

Be sure that all the following documents are attached to the employment contract. Arrange them chronologically as they are listed here. For re-contract, follow the procedure in express employment contract processing. For change employer, submit them to Counters 8 and 9. (Siguraduhin na ang lahat na sumusunod na dokumento ay nakakabit sa kontrata ng paggawa. Iayos ng sunod-sunod batay sa nakalista sa ibaba. Para sa Re-Contract, sundin ang madaling pamamaraan sa pagproses ng kontrata. Para sa Change-Employer, pumunta sa Counter 8 at 9 para isumite ang  kontrata.)

PERIOD TO PROCESS EMPLOYMENT CONTRACT

Panahon sa Pagproses ng kontrata sa empleo

-   RE-CONTRACT (within 45 days before visa expires)

                      PAG-ULIT NG KONTRATA (sa loob ng 45 araw bago matapos ang bisa)

-   CHANGE-EMPLOYER (within 30 days before visa expires)

                      PAGLIPAT O PAGBABAGO NG AMO (sa loob ng 30 araw bago matapos ang bisa)

 

A.   FINISHED CONTRACT

 Natapos na kontrata

      1.   Old Contract

      Lumang kontrata

      2.   New Contract (Revised 10/2003 ) completely filled-up. All erasures in the contract must be countersigned both by the employer and helper

      Bagong kontratang (Binago 10/2003) may kumpletong kasagutan. Lahat ng pagbura o pagbago ay kailangang may pirma pareho ng amo at kasambahay 

      3.   Photocopy of Passport and valid visa

      Poto-kopya ng Pasaporte at balidong bisa

      4.   Photocopy of Worker’s HK ID card

      Poto-kopya ng HK ID card ng manggagawa

      5.   Photocopy of Employer’s HK ID card

      Poto-kopya ng HK ID card ng amo

      6.   OFW Information Sheet with completely filled-up (obtain from the guard). You may download it.

      Sipi ng impormasyon ng OFW na may kumpletong sagot (makukuha sa guwardiya). Maaring i-download ito.

 

B.   PRE-TERMINATED CONTRACT (Transfer/Change Employer)

Di natapos na kontrata (Paglipat/Pagbago ng Amo)

      1.   Old Contract

      Lumang kontrata

      2.   New Contract (Revised 10/2003 )

      Bagong kontrata (Binago 10/2003)

3.   Original Passport w/valid visa

      Orihinal na Pasaporte na may balidong bisa

      4.   Photocopy of Worker’s HK ID card

      Poto-kopya ng HK ID card ng manggagawa

      5.   Photocopy of Employer’s HK ID card

      Poto-kopya ng HK ID card ng amo

      6.   Photocopy of ID 522 (Appointment letter from the Hong Kong Immigration Department)

      Poto-kopya ng ID 522 (Sulat ng Paghirang mula sa Kagawaran ng Imigrasyon ng Hong Kong)

      7.   OFW Information Sheet completely filled up (obtain from the guard). You may download it.

      Sipi ng impormasyon ng OFW na may kumpletong sagot (makukuha sa guwardiya). Maaring i-download ito.

 FOR RENEWAL/RE-CONTRACT ONLY
 
Secure OFW Information Sheet from the Consulate guard or you may download it. Fill up completely the Information Sheet
Kumuha ng OFW Information Sheet sa guwardiya ng Konsulado o maaring i-download ito. 
Attach completely filled-up OFW Information Sheet to the Employment Contract and other required and/or supporting documents.
Ikabit ang may sagot na OFW Information Sheet sa kontrata kasama ang iba pang kailangang dokumento
Give the above-mentioned documents to the Cashier together with the exact amount of HK$297.50 as processing fee. Get the Original Receipt (O.R.-white).
Ibigay ang mga nasabing dokumento sa Kahero kasama ng eksaktong halagang HK$297.50 bilang bayad sa pagproses. Kunin ang Orihinal na Resibo (OR-puti)
 
Bring the O.R. after 3 working days to Counter 13 for the release of employment contract and other documents.
Dalhin ang OR pagkatapos ng 3 araw na may pasok sa Counter 13 para kunin ang kontrata at iba pang dokumento.
