Requirements for Notarization and Authentication of Documents

1. Affidavits, Sworn Statements, Certification, etc.

  • Personal appearance of person/s executing/signing the document 
  • Identification (HK ID or Passport) 
  • Extra Copy of Document

2.  Acknowledgement of Instruments (Deeds, Power of Attorney, Real Estate Mortgage, Letters, etc.)

  • Personal appearance of person/s signing the document/s, including two witnesses (signature over printed name)
  • Identification (HK ID or Passport) 
  • Extra Copy of Document

3.  Affidavit of Consent and Support (for minors with one or both parent/s in HK or Macao)

  • Personal appearance of parent/s
  • Identification (HKID or MCID [front and back] and Passport [data/visa]) – 2 copies each
  • Form (please see downloadable forms)

Note: Per new DSWD guidelines (www.dswd.gov.ph), minors traveling abroad whose parents are living abroad (residents of HK/Macao SAR) or are in the foreign service need not secure the Affidavit of Consent and Support (AOCS) and the DSWD Travel Clearance provided, that the minor holds a valid dependent's or permanent resident visa and has a valid passport. If not, the AOCS is being required by the DFA for the passport application processing. Also, minors traveling with either parent or solo parent or legal guardian are exempted from securing the Affidavit of Consent and Support (AOCS) from the Consulate and the Parental Travel Permit (PTP) from the DSWD.

4.  Affidavit of Support (for 18 y/o & above /minors w/o parents in HK or Macao)

  • Personal appearance of person/s signing the document
  • Identification (HKID or MCID [front and back] and Passport [data/visa]) – 2 copies each
  • Form (please see downloadable forms)

5.  Affidavit of Consent and Support (WEG) (for foreign passport holders below 15 years old)

  • Form (please see downloadable forms)
  • Personal appearance of parent/s
  • present parent/s’ HK ID or passport
  • 2 photos per child
  • form available at the Visa Section (counters 6 & 7)

6. Authentication 

      a.  Documents issued in the Philippines for use in HK or Macao

  • Original document authenticated by authorized authentication offices in the Philippines (issuing agency / DFA)
  • Photocopy of document

      b. Documents notarized in Hong Kong for use in the Philippines

  • Original notarized document and photocopy thereof
  • Proof of authority of the notary public

      c.  Documents issued by Hong Kong Government official(s)

  • Original document and photocopy thereof

      d.  Documents issued in Macao for use in the Philippines

  • Original document notarized by a Macao notary public and photocopy thereof
  • Proof of authority of the notary public
  • documents issued by the Macao Govt  (ex. birth/marriage/death certificates, certificate of no criminal conviction, certificate of registration of trade mark, etc.)
  • original documents with English translation and photocopy thereof

7.  SHIPMENT OF HUMAN REMAINS TO THE PHILIPPINES

      A. FROM HONG KONG SAR TO THE PHILIPPINES

  1. Certified Copy of an Entry in a Register of Deaths issued by the Births and Deaths Registry, Hong Kong SAR or Certificate of Fact of Death issued by the Coroner’s Court, Hong Kong;

  2. Duly notarized certificate issued by the Mortuary or Funeral Director (please see attached sample format – 2nd paragraph should be included if no separate Certificate of Non-contagious Disease is issued);

  3. Transit Permit issued by the Immigration Department, Births and Deaths General Register Office, Hong Kong; and

  4. Fee: HK$212.50

Note:

All the above documents (one set original, 3 sets copies) must be submitted to the Consulate prior to the issuance of Consular Mortuary Certificate and shipment of the remains to the Philippines. (For remains originating in Shenzhen or Guangzhou to be brought to the Philippines via Hong Kong, a Death Certificate duly authenticated by the Consulate General in Guangzhou must be presented together with the other requirements - items 2-4 above)

The original passport of the deceased must be submitted for cancellation.

Sealing of casket or urn by a Consular Officer is no longer required.

     B. FROM MACAO SAR TO THE PHILIPPINES

  1. Certificate of Death (original or CTC issued by the Civil Affairs Registry, Macao SAR);

  2. Bulletin of Death issued by the Registry Office of Marriages and Deaths, Macao SAR;

  3. Sanitary Medical Certificate issued by the Health Bureau, Macao SAR or Certificate of Non-contagious Disease;

  4. Duly notarized certificate issued by the Mortuary/Funeral Director (please see attached sample format);

  5. Transit Permit issued by the Public Security Police Force, Macao SAR; and

  6. Fee: HK$212.50

Note:

All the above documents (one set original, 3 sets copies and with English translations) must be submitted to the Consulate prior to the issuance of Consular Mortuary Certificate and shipment of the remains to the Philippines. (For remains originating in Shenzhen or Guangzhou to be brought to the Philippines via Macao, a Death Certificate duly authenticated by the Consulate General in Guangzhou must be presented together with the other requirements - items 3-6 above)

The original passport of the deceased must be submitted for cancellation.

8.  Requirements for Bringing the Urn Containing the Cremated Ashes of the Deceased to be Buried/Deposited in the Philippines:

  1. Documents for Authentication with the Philippine Consulate
    1. Certificate of Cremation
    2. Authorization to remove urn containing the ashes of the deceased
    3. Fee: HK$212.50

    Note:

    Please submit the original and photocopies of the above documents prior to shipment of the urn

    Original Death Certificate-relative may apply for authentication to support their claims for insurance, etc. Fee is HK$212.50

9.  Application for Waiver on the Use of Philippine Flag Vessel

  • Duly accomplished form in five (5) sets (please see downloadable forms)
  • Photocopy of packing list
  • Photocopy of bill of lading 
  • Application letter addressed to the Consul General

10.  Establishing Regional Headquarters in the Philippines

  • Letter-request on the issuance of a certificate
  • Copy of HK Business Registration  
  • Copy of Memorandum and Articles of Association
  • Company Profile (including business address, nature of business, members of Board of Directors and list of affiliates in Asia-Pacific) 
  • Board resolution authorizing the establishment of the regional headquarters in the Philippines

11.  Requirements for the Issuance of the Provisional Certificate of RP Registry (PCPR) 

  • Request letter from authorized agent or party
  • Business registration certificate of agent
  • Certificate of registry of the vessel
  • Bureau Veritas classification or class certificate and Survey Form
  • Ship's particulars (size, tonnage, building material, speed, etc)
  • MOA between exporter and importer
  • Authority from Philippine MARINA (thru OSEC/ISS) for vessel to be issued a PCPR

12.  Renunciation of Philippine Citizenship

  • Personal appearance
  • All RP passports (old and new) with 3 photocopies each (impt. pages only)
  • Affidavit of renunciation - 2 copies (click here to download)
  • Birth Certificate – original & 3 copies
  • HK ID/Macao ID – original & 3 copies
  • Letter from HK/Macao Immigration – original & 3 copies
  • Marriage Certificate (if married) – original & 3 copies
  • Fees: HK$212.50 (for AOR notarization); HK$212.50 (for Certificate of Renunciation)
  • Processing period:  2 working days

13. For Certification to Avail of the Exemption from Withholding Tax on Interest Income Derived from Foreign Currency Deposit Units (FCDU) Maintained by Non-Residents in RP as Required by BIP Reg. No. 10-98

  • Letter request
  • Letter from the bank
  • Original passport and HK ID with photocopies (visa page)

14. For Certification to Avail of Tax Exemption for Personal/Household Belongings for Presentation to RP Customs Authorities (Expats returning to RP for Good)

  • Letter request stating that you/your family will reside permanently in RP (state number of years of stay/work in HK)
  • Original passport and HKID – with copies
  • Copy of Air way Bill / Bill of Lading
  • Copy of packing list
  • Letter from company – stating no. of years of service and position

15.  Guidelines/Procedure For Importing Live Pet Animals

  • DOGS AND CATS

Upon arrival at the ports of entry at Airport or Seaport, documents to be presented to the Veterinary Quarantine Personnel:

  1. Import Permits - Priorly issued by the Bureau of Animal Industry , Department of Agriculture, Visayas Avenue, Diliman, Quezon City, Philippines.

  2. Animal Health Certificate and Veterinary Health Certificate - duly authenticated by the Philippine Consulate General, Hong Kong SAR.

  • PROCEDURE IN APPLYING FOR IMPORT PERMITS

    1. Write the Director of Bureau of Animal Industry (above address) for issuance of Import Permit. In the application letter, indicate the number of animals to be imported, the sex, age and breed; and also the possible date of arrival, mode of transport - aircraft / flight number.

    1. Upon receipt of the application, the BAI will process and issue the Import Permit and immediately send the same to the applicant by mail or by facsimile.

    2. Payment of Import Permit Issuance Fee of P100.00 and Quarantine  Inspection Fee or Landing Permit Fee of P250.00 will be made to the Quarantine Office upon arrival at port of entry. (Fee are subject to change without prior notice)

  1. ADDITIONAL DOCUMENTS TO BE SUBMITTED FOR PETS ORIGINATING IN HKSAR

    1. VETERINARY HEALTH CERTIFICATE - issued by the Veterinary certifying the health worthiness of the animals which includes vaccination and endorse to the HK Gov't., Agriculture, Fisheries and Conservation Department and apply for an Animal Health Certificate.

    2. ANIMAL HEALTH CERTIFICATE - issued by the Agriculture, Fisheries & Conservation Department.

  1. HEALTH CERTIFICATE FOR PETS - issued by the Veterinary Officer of the INSTITUTO PARA OS ASSUNTO CIVICOS MUNICIPALS certifying the health worthiness of the animals which includes vaccination.
