Amendments to data appearing in a Philippine passport shall be allowed only in the following cases:

1. Amendment of a female passport holder's name due to marriage
(a) Requirements for those married in the Philippines
Copy of marriage contract duly authenticated by the National Statistics Office (NSO) 
Copy of female applicant's certified true copy of birth certificate
(b) Requirements for those married in Hong Kong SAR
Three (3) copies of marriage contract/certificate issued by the HK Marriage Registry (bring original) 
Three (3) copies of authenticated birth certificate (bring original)
Three (3) copies each of couple's passports (showing name and photo)
Four (4) copies of duly accomplished Report of Marriage form available here or at the Information Counter of the Consulate
(c) For marriages solemnized elsewhere
Three (3) copies of authenticated birth certificate (bring original) 
Three (3) copies each of couple's passports (showing name and photo)
Four (4) copies of duly accomplished Report of Marriage form available here or at the Information Counter of the Consulate
Three (3) copies of Marriage Contract authenticated by the nearest Philippine Consulate/Embassy where the marriage was solemnized.
2. Amendment of female passport holder's name due to death of spouse, 
annulment of marriage or divorce initiated by a foreign spouse
(a) Due to death of spouse
Copy of authenticated death certificate of husband 
Copy of previous marriage contract
Copy of authenticated birth certificate
Copy of authenticated court order of presumptive death (where applicable)
(b) Due to annulment of marriage
Copy of previous marriage contract
Copy of Certificate of Finality and Court Order (duly authenticated by Malacanang Legal Office and Department of Foreign Affairs, Manila)
Copy of authenticated birth certificate
(c) Due to divorce initiated by a foreign spouse
Copy of court decision on divorce
Copy of authenticated birth certificate
(d) Divorce among Filipino Muslims
Copy of divorce decree from Shariah Court
Joint birth affidavit executed by two Imams who actually witnessed the divorce declaration
3. Amendment of Child's name who is legitimated by virtue of subsequent 
marriage of parents
Copy of previous birth certificate- Copy of authenticated amended birth certificate
Copy of authenticated marriage contract of parents
