| Filipino Tourists who lost their passports in Hong Kong SAR shall be issued a travel document in lieu of a passport. They should submit the following documents for the issuance thereof: |

Duly accomplished application form available here or at the Philippine Consulate |

Photocopy of Police Report |

Affidavit of Loss (in duplicate; form obtainable at the Information Counter of the Consulate) |

Photocopy of airline certification on arrival in HK SAR |
 |
Copy of any identification card issued in the Philippines or previous Philippine passport Copy of ticket/ certification of confirmed return flight to the Philippines |

Four (4) photographs

Replacement
passport may be issued only after the lapse of FIFTEEN (15)
working days from the date of submission of the Affidavit of

Applicants from Macau
may submit their documents to the Consulate through their authorized representatives. A letter addressed to Macau/Hong Kong Immigration will be issued to enable them to proceed to the Consulate to file their application for replacement passports in person. |

Photos should show applicant in decent attire, with sleeves and preferably with collar. |

Women should not wear veils, except for Muslims and nuns. |

Passport photos taken at the MTR or Photo-me are not acceptable. |
