1. Passport Requirement

    1. Duly accomplished passport application form (available here or at the Philippine Consulate) MUST BE PRINTED IN LEGAL SIZE FORM (8.5in × 14in).
    2. Latest Passport and photocopy of first and last page with latest visa. If amended, include page with amendment.
    3. Photocopy of HK ID (back to back), if resident. Copy of Macau visa/ID, in case of Macau resident.
    4. Photocopy of valid passport of parents, if minor.
    5. Three (3) photos (please read photo requirements below)
    6. Personal appearance of the applicant is required.

  2. Registration of babies born in Hong Kong

    1. Duly accomplished application form (available here or at the Philippine Consulate) MUST BE PRINTED IN LEGAL SIZE FORM (8.5in × 14in).
    2. 3 copies of birth certificate
    3. 3 copies of parent's marriage contract. If married in the Philippines or in other countries, marriage contract must be authenticated by NSO. If married in Hong Kong, copy of Report of Marriage.
    4. If not married, authenticated birth certificate of mother together with Affidavit of Illegitimacy. Affidavit of Recognition/Admission of Paternity and Authority to use surname of Father to be submitted prior to report of birt of child.
    5. 3 copies of parent's passports
    6. 1 copy of Report of Birth (Form provided by this Office)
    7. Affidavit of Late Registration of Birth for births reported after twelve (12) months from occurrence (effective Jan 2008).
    8. 3 photos (photo requirements shall apply if applying for a passport)

  3. For Seamen

    1. Copy of Seaman's Book
    2. Three (3)photos
    3. Certification from local agency
    4. Copy of Crew list
    5. Copy of Philippine Coast Guard ID

  4. For Naturalized Filipinos

    1. Naturalization papers (copy and original)
    2. Requirements for renewal of passport shall apply

  5. In case of Lost Passports

    After submission of documentary requirements, there is a prescribed clearing period of 15 days prior to issuance of replacement. All passports not issued in PCG Hong Kong shall be replaced only upon receipt of confirmation telex from DFA-Manila.

    1. Tourist (for issuance of Travel Document only)
      1. Affidavit of Loss (2 copies)
      2. Police Report
      3. Airline Certification of Arrival in Hong Kong SAR
      4. copy of previous passport/Philippine issued ID
      5. Copy of Ticket/Certification of confirmed flight to the Philippines.
      6. Four (4) photos

    2. Contract Workers/Residents
      1. Affidavit of Loss (2 copies)
      2. Police Report
      3. Copy of lost passport (if not available, authenticated birth certificate)
      4. Authenticated marriage contract/Report of marriage if lost passport is under married name.
      5. Copy of valid employment contract and Hong Kong ID
      6. Photo requirements and personal appearance shall apply

  6. Amendments/Report of Marriage

    Only two kinds of amendment to passport are allowed:
    1. Female applicants who wish to adopt husband's surname
    2. Female applicants who wish to revert to maiden name due to death of spouse/annulment of marriage.

    1. For those married in the Philippines
      1. NSO authenticated marriage contract
      2. NSO authenticated birth certificate

    2. For those married in Hong Kong SAR
      1. Duly accomplished application form (available here or at the Philippine Consulate) MUST BE PRINTED IN LEGAL SIZE FORM (8.5in × 14in).
      2. 3 copies of marriage contract
      3. 3 copies of NSO birth certificate of wife
      4. 3 copies of passports of husband and wife
      5. 1 copy of Report of Marriage (Form provided by this Office)
      6. Personal appearance
      7. Affidavit of lat registration for those marriages reported after twelve (12) months from occurrence (effective January 2008)

    3. For marriage solemnized elsewhere other than in Hong Kong SAR/Philippines
      Please submit item nos2, 3, 4 mentioned in item VI letter B together with the marriage contract authenticated by the Philippine Mission nearest to the place where the marriage was solemnized.

      If applying for the amendment to adopt husband's surname, please submit NSO authenticated Report of Marriage.

  7. Married female applications reverting to maiden name

    1. For Widows
      1. NSO authenticated death certificate of husband
      2. NSO authenticated birth certificate of wife
      3. NSO authenticated copy of previous marriage
      4. In case of presumptive death of husband, Court Order of Presumptive Death duly authenticated by DFA

    2. For Annulled Marriages:
      1. NSO authentication marriage contract with annotation on annulment
      2. Certified true copy of Court Order and Certificate of Finality duly authenticated by DFA.
      3. NSO authenticated birth certificate of wife

    3. For Divorcees:
      1. Court Decision on Divorce Petition (petitioner should be the foreign spouse. In case petitioner is the Filipino spouse, item no. VII letter b shall be required)
      2. NSO authenticated birth certificate of wife

    4. Divorce among Filipino Muslims
      1. Divorce Decree from Shariah Court
      2. Joint Affidavit executed by two Imams who witnessed the divorce declaration

  8. The Consulate reserves the right to require additional documents deemed necessary.

Passport Photo Specifications
Photos, except when used for Travel Documents, must be taken using royal blue background showing applicant's face and prominent features clearly. Photos should show applicants in decent attire, with sleeves and collar.

Applicants maybe allowed to wear eyeglasses provided the eyes are distinctly shown. Women are not allowed to wear veil except in cases of Muslims and nuns. Long hair and earring are not allowed for men.

Photos taken at the MTR or Photo-Me are not acceptable.

SCHEDULE:
Consular service is from Sunday to Thursday only. Passports are released after 20 working days.

Processing Time:9:00am to 4:00
Releasing Time:9:30am to 1:30pm

IMPORTANT REMINDERS:
  1. After processing, please pay immediately at the cashier.
  2. Declare lost, valid or expired passport to avoid delay in the issuance of a new one.
  3. Passports are released only to applicants or to immediate family members (spouse, sibling, parent, child of legal age)
  4. Check all data in the passport upon release. Passports found to have scripting errors will be replaced gratis if presented for correction within 6months from date of issuance
  5. Passports unclaimed after 6 months from date of issuance will be cancelled per Department Order 37-03.
